Building a Healthier Future: Nevada County’s Community Health Improvement Plan

When you think about community health, it’s more than just doctor visits and flu shots. It’s the air we breathe, the food on our tables, the stress in our lives, and the roof over our heads. Nevada County is looking at the big picture with its 2025–2027 Community Health Improvement Plan (CHIP)—a roadmap that goes beyond band-aids and digs into the root causes of what keeps our community well (or unwell).

What is the CHIP?

The CHIP stems from a larger Community Health Assessment (CHA) that analyzed local data, health disparities, climate risks, and community feedback. Together, these two documents give Nevada County the chance to set clear priorities and put strategies in place to make a real difference in the lives of residents.

At its heart, the CHIP carries a strong vision: an engaged, diverse, connected, healthy, and happy community. Those aren’t just buzzwords. They’re values backed up with commitments to equity, inclusion, acceptance, compassion, collaboration, and trust.

The Four Big Priorities

Healthy Living & Health Equity
The plan pushes for better access to preventive care, nutrition, and exercise resources. It’s about making healthy living less of a privilege and more of a right for everyone.

Behavioral Health
Mental health and substance use aren’t swept under the rug. Suicide prevention, addiction support, and early intervention are top priorities—recognizing that a community can’t thrive without emotional well-being.

Social Determinants of Health
Housing, income stability, education, connectedness, and even climate resilience are all being addressed. Because health doesn’t stop at the clinic door—it’s shaped by where we live, work, and play.

Climate & Environment
With wildfire smoke, extreme heat, and changing weather patterns, climate change is a health issue here in Nevada County. The CHIP calls for protecting vulnerable populations and building resilience to environmental hazards.


This isn’t just theory. The CHIP lays out practical steps like:

Collecting and analyzing data to uncover gaps in care.

Engaging directly with under-represented groups to shape decisions.

Expanding behavioral health services despite workforce shortages.

Tackling climate-driven health risks head-on.

Challenges remain—like sustainable funding, rural access, and staffing—but the framework is there to keep Nevada County moving forward.

Health isn’t an individual journey. It’s a community effort. By focusing on equity, behavioral health, social supports, and climate resilience, Nevada County is setting a course toward a healthier, stronger future for us all. And that’s worth paying attention to.

The 2025–2027 CHIP is a reminder that community health is multi-layered. It’s not just about patching holes, but weaving a stronger net that catches everyone—no matter their income, background, or zip code. If we can hold ourselves accountable to these goals, Nevada County has the potential to become not only healthier, but more connected and compassionate too.
